A recent FDA alert, along with new labeling, warns health professionals about the possibility of QT prolongation and Torsades de Pointes (TdP) in patients treated with the antipsychotic drug Haldol (haloperidol). The risk appears to be higher when the drug is administered intravenously or in higher doses than recommended. Injectable haloperidol is approved only for intramuscular administration, but it’s sometimes used intravenously to treat severe agitation. FDA is advising particular caution if the drug is used in patients who have other QT-prolonging conditions, including electrolyte imbalance. Caution should also be used in patients taking drugs known to prolong the QT interval, and in those who have underlying cardiac abnormalities, hypothyroidism or familial long QT syndrome. FDA also recommends ECG monitoring if haloperidol is given intravenously.

5 Warning Signs Your Hashimoto’s Hypothyroidism Is Being Mismanaged

“Your thyroid levels are in the normal range now, take these antidepressant’s, they will help with your depression.” These may be some of the most dangerous words a low thyroid sufferer will ever hear.

According to the Endocrine Society, upwards to 80 or 90 percent of hypothyroidism in the United States, is caused from an autoimmune mechanism called Hashimoto’s Autoimmune Thyroid. This means the cause of the hypofunction is the immune system attacking your own gland.

Literally the immune system slowly chews up the thyroid gland and you lose normal thyroid hormone output. Over time your hormone levels will slowly decrease because it continually loses cells from the immune attack.

Here are 5 Warning Signs that you may have hypothyroidism from an immune mechanism, and that the treatment your are currently receiving is not adequate or complete enough for your particular condition:

1) The dose of your thyroid replacement hormone continually goes up over time.

If you are continually having your thyroid medication increased, this means that your are losing more and more thyroid function. Your immune system is most likely the culprit. Hormone replacement does not address the continual immune attack against the gland. If your doctor is not addressing the immune attack, you are going to continue to suffer.

2) You still suffer with hypothyroid symptoms, yet your doctor tells you that your thyroid is now “normal”.

If your latest labs show that your hormone levels are in the “normal range”, yet you continue to suffer with the same symptoms, you are most likely not getting the proper management of your condition.

This is a huge sign that you have Hashimoto’s disease. The proteins, called cytokines, that are used by the immune system to communicate amongst each other, block the hormone receptor sites. This means you can have normal hormone levels showing on a lab report, and still not have a normal thyroid metabolic responses. Working on the immune system is the only way to fix this problem.

3) Your doctor never checked for antibodies against the gland.

If you suffer with hypothyroidism, then you have a high probability of having Autoimmune Thyroid. If your doctor didn’t bother to check antibody levels, or refuses to check antibody levels, you are most likely not receiving the proper treatment. The two most important antibodies to check are Thyroid Perioxidase Antibodies (TPO Ab) and Thyroglobulin Antibodies (TBG Ab).

4) Your doctor continually checks your antibody levels.

This may seem counter-intuitive since I just mentioned that you should have your antibody levels checked. I include this because, replacement hormones do not affect the immune system. It makes no sense to continually check antibodies if the doctor is not doing any treatment to affect the immune system. If your doctor is trying to decrease your antibody levels with replacement therapy, you are not getting the proper treatment. Watch for this. It is a sign that your doctor really doesn’t understand the mechanisms causing your problem.

5) You begin to get individual prescriptions for various thyroid symptoms.

If you are getting treated with antidepressants for hypothyroid based depression, it means that the replacement hormone treatment is most likely not working. With Hashimoto’s you can have normal thyroid hormone levels and still suffer with symptoms, including depression. Treatment needs to be focused on immune modulation, if you want to fix the actual cause of the problem.

If you suffer with hypothyroidism, and you live in the United States, then the most likely reason you have this condition is from an autoimmune reaction called Hashimoto’s Disease. Treating this condition with replacement hormone, while ignoring the immune component, is like treating a slow bleed with blood transfusions and failing to stop the site of the bleed. Without immune modulation then the viscious cycle of thyroid gland destruction, by the immune system, will continue. You can use the warning signs outlined as a guide to help you determine if you are receiving the proper management of your hypothyroid condition.

What Is Hashimoto’s Disease?

Hashimoto’s disease (by Japanese surgeon; Hashimoto, Hakaru 1881-1934) is a chronic thyroiditis. It is characterized by the production of autoantibodies that attack the thyroid. This will eventually cause a lack of thyroid hormone, thyroid fibrosis, and infiltration of thyroid tissue by lymphoid tissue.
Hashimoto’s disease is also a common cause of a goiter. A goiter is an enlargement of the thyroid gland. It becomes clearly visible as a swelling on the front part of the neck because the thyroids tends to grow 2 to 5 times bigger than normal.
Hashimoto’s disease is also called Hashimoto’s struma, Hashimoto’s thyroiditis, or struma lymphomatosa.

The thyroid gland secretes two hormones controlling the usage of energy and the body temperature. To determine if the thyroid gland is working properly, different hormone levels can be measured in the blood. The pituitary gland secretes a hormone called TSH (thyroid-stimulating hormone). TSH level is clearly elevated when the thyroid gland is not functioning properly. Also an iodine absorption test may be performed to show too low uptake of iodine, which may indicate hypothyroidism.

If you suspect that your thyroid is not active enough, you can do a thyroid self-test in the morning. Stay in bed after you awake and take your temperature from under your arm. Try to stay very still and quiet for fifteen minutes. If your temperature is 97.6F or lower for 5 days, you can ask your professional health care provider if there is a reason to test the condition of your thyroid.

Symptoms of Hashimoto’s disease include fatigue, loss of appetite, weight gain, sensitivity to cold, muscle cramps, depression, fertility problems, painful premenstrual periods, muscle weakness, dry and scaly skin, yellowish coloration in the skin, yellowish bumps on the eyelids, hair loss, constipation, persistent infections, swollen eyes, and milky discharge from breasts.

Hashimoto’s disease is a rare disease and it is more common among women than men. Usually it appears in the age range of 30-50. About 10-30% of people suffering from Hashimoto’s disease will develop hypothyroidism. The treatment is usually replacement of the thyroid hormone for the rest of the patient’s life. With You Or Against You? How Your Body Works Against You in Hashimoto’s Thyroiditis

When your immune system starts to work against your own tissues, what you have is a condition called an autoimmune disease. The first autoimmune disease to be classified as such is Hashimoto’s thyroiditis or Hashimoto’s disease. What is Hashimoto’s Thyroiditis?

Hashimoto’s thyroiditis is an autoimmune disease wherein the T-cells produced by the immune system to fight against what the immune system perceives as foreign — and therefore, a threat — starts to attack the cells of the thyroid gland itself. This causes irritation and swelling of the thyroid gland, rendering it unable to produce the hormones necessary to maintain the body’s metabolism.

Hashimoto’s thyroiditis was named after the Japanese physician Hashimoto Hakaru, who first described the disorder in a German publication in 1912. Since then, it has become one of the most common causes of hypothyroidism in areas with sufficient iodine sources.

Symptoms — What to Look Out For

Most of the symptoms of Hashimoto’s thyroiditis is associated with the low concentrations of thyroid hormone in circulation in the blood. These symptoms include fatigue and extreme somnolence, constipation, mental sluggishness, decreased cardiac output, intolerance to cold, and sometimes, a slight gain in weight. There could also be failure of the trophic functions of the body as evidenced by scaly skin, depressed hair growth, and the development of a husky voice.

There could also be development of a swelling of the thyroid gland known as a goiter. Due to the low levels of thyroid hormone, your pituitary gland keeps secreting thyroid-stimulating hormone or TSH to stimulate the thyroid gland to activity. Because of this constant stimulation and the thyroid gland’s inability to secrete thyroid hormone, the thyroid glands starts to swell instead.

Diagnosis — How Your Doctor Knows You Have Hashimoto’s Thyroiditis

There are several blood tests that may hint at a patient with Hashimoto’s thyroiditis. These are the measurement of the concentrations of thyroid hormone present in circulation. Total thyroxine, free thyroxine, and triiodothyronine are commonly used to screen for thyroid disorders.

Total thyroxine is the amount of thyroxine released in the bloodstream, both bound and free. Thyroxine is commonly found bound to a protein called thyroxine-binding globulin or TBG. Unless it is bound to TBG, thyroxine can exert its effect on the tissues of the body. Triiodothyronine is the other hormone produced by the thyroid gland and can cause an effect on the tissues faster than thyroxine. Most of the thyroxine released from the thyroid gland is eventually converted to triiodothyronine.

If your doctor finds your thyroid hormone levels to be lower than usual, he might suspect you have Hashimoto’s thyroiditis and request several more blood tests to confirm it. These tests include tests for autoantibodies that attack your thyroid gland like antibodies against thyroglobulin and thyroid peroxidase. Most people with Hashimoto’s thyroiditis have high levels of these antibodies.

Other tests include fine needle aspiration biopsy, nuclear medicine scans, and ultrasound of the thyroid gland. Patient’s with Hashimoto’s thyroiditis often show lymphocytes and macrophages in FNA biopsies.

Treatment — How Do You Recover From Hashimoto’s Thyroiditis

Patients with Hashimoto’s thyroiditis may be treated with small amounts of thyroid hormone to make up for the lack of the hormone. In cases when the goiter poses a problem in swallowing or breathing or is getting to be a cosmetic disaster, levothyroxine may be given to shrink the thyroid. If this intervention does not work, surgery may be recommended.

When your own immune system starts to work against you, there is very little treatment that could ensure complete recovery. However, some studies show that treatment with levothyroxine may significantly reduce the lymphocytes and macrophages responsible for Hashimoto’s thyroiditis.
