Types of Thyroid Surgery
There are three main types of thyroid surgery : -
Total Thyroidectomy – Complete Removal of the Thyroid
This is the most common type of thyroid surgery, and is often used for thyroid cancer, and in particular, aggressive cancers, such as medullary or anaplastic thyroid cancer. It is also used for goiter and Graves’/hyperthyroidism treatment.
Subtotal/Partial Thyroidectomy – Removal of Half of the Thyroid Gland
For this operation, cancer must be small and non-aggressive — follicular or papillary — and contained to one side of the gland. When a subtotal or partial thyroidectomy is performed, typically, surgeons perform a bilateral subtotal thyroidectomy which leaves from 1 to 5 grams on each side/lobe of the thyroid. A Harley Dunhill procedure is also popular, in which there’s a total lobectomy on one side, and a subtotal on the other, leaving 4 to 5 grams of thyroid tissue remaining.
Thyroid Lobectomy – Removal of Only About a Quarter of the Gland
This is less commonly used for thyroid cancer, as the cancerous cells must be small and non-aggressive…..
How It’s Done
Both types of thyroidectomy are usually done under general anesthesia. However, if general anesthesia is too risky for a patient, local or regional anesthesia may be used to permit the patient to remain awake during the procedure. An intravenous (IV) line will be inserted into one of your veins to deliver fluids and medications….
Conventional thyroidectomy
In a conventional thyroidectomy, a 3- to 4-inch incision will be made through the skin in the low collar area of your neck (the lower front portion of your neck, above the collarbones and breast bone). Next, a vertical cut will be made through the strap-like muscles located just below the skin, and these muscles will be spread aside to reveal the thyroid gland and other deeper structures. Then, all or part of your thyroid gland will be cut free from surrounding tissues and removed….
Endoscopic thyroidectomy
A viewing instrument called an endoscope and small surgical instruments will be inserted into your neck through three or four small incisions. Each incision is about 3 millimeters to 5 millimeters long (less than ? inch). Then the surgeon will use a tiny camera on the endoscope to guide the instruments and remove your thyroid tissue. At the end of the procedure, your neck incisions will be closed with tiny stitches or surgical tape….
The Surgical Procedure
In the surgery, the surgeon will cut a 3- to 5-inch incision across the base of your neck in front. The skin and muscle are pulled back to expose the thyroid gland. The incision is usually made so that it falls in the fold of the skin in your neck, making it less noticeable.
Blood supply to the gland is “tied off,” and the parathyroid glands are identified (so that they can be protected). The surgeon then separates the trachea from the thyroid, and removes all or part of the gland….
Possible complications include : -
Hemorrhage (bleeding) beneath the neck wound ?
If this occurs, the wound bulges and the neck swells, possibly compressing structures inside the neck and interfering with breathing. This is an emergency….

A thyroidectomy is an operation that involves the surgical removal of all or part of the thyroid gland. Surgeons often perform a thyroidectomy when a patient has thyroid cancer or some other condition of the thyroid gland (such as hyperthyroidism). Other indications for surgery include cosmetic (very enlarged thyroid), or symptomatic obstruction (causing difficulties in swallowing or breathing)…
The issue of a subtotal/partial, vs. total thyroidectomy is controversial. Some practitioners prefer to perform a partial thyroidectomy whenever possible, believing that they will leave behind enough thyroid tissue to prevent hypothyroidism. (A total thyroidectomy has nearly a 100 percent chance of causing hypothyroidism).
The risk of hypothyroidism with subtotal thyroidectomy is, however, quite high, and some experts say that more than 70 percent of patients receiving a subtotal thyroidectomy will become hypothyroid. Since one of the main reasons for subtotal thyroidectomy is to prevent hypothyroidism, and that goal is achieved in only a minority of cases, experts increasingly believe that there is no added benefit to subtotal thyroidectomy, and are more routinely recommending a total thyroidectomy.
Surgical removal of the thyroid is necessary in some situations where a goiter becomes life-threatening or in the presence of some cancers.
Post-thyroidectomy treatment with thyroid hormone is routinely done to prevent a condition known as myxedema. The lack of circulating thyroid hormone in the body gives rise to a series of signs and symptoms which represent a severe form of hypothyroidism (underactivity of the thyroid gland)…

When is Thyroid Surgery Necessary
Many thyroid problems can be effectively treated without the necessity of having to undertake a surgical procedure but there are instances where a doctor might decide that thyroid surgery is the best possible course of action – or perhaps even the only course open.
If a patient has a thyroid nodule and it has continued to grow despite possible fluid removal or the prescribing of thyroxine, a doctor might conclude that surgery is necessary. Similarly, if the nodule is causing the patient pain or undue anxiety. Many doctors consider that once a nodule has a diameter in excess of 4 centimetres it should be operated upon. The Hot nodules (those stimulating the production of the thyroid hormone) are generally treated by either radio-iodine or surgery, and some patients will opt for the surgical procedure because of their concerns
over radio-iodine therapy.
Indications are that the risk of developing hypothyroidism as a result of having a nodule surgically removed for this reason is very low. Likewise, most multinodular goitres can be treated without recourse to surgery. Again, though, there are times when surgery becomes necessary. For example, surgery is indicated if the goitre continues to grow despite treatment; if the goitre has become toxic; if a patient has developed an unsightly looking lump and wants it removed for predominantly cosmetic reasons; if the goitre is substernal – in other words situated in
the lower part of the isthmus of the thyroid gland and difficult to locate and observe – and is considered to be most suitable for surgical removal; and finally if the goitre has resulted in compression of the trachea, which can lead to respiratory problems or infection, or has culminated in arterial compression, which can cause cerebral hypoperfusion and perhaps induce a stroke.
The most common reason for thyroid surgery to be carried out, however, remains thyroid cancer. Although on these occasions it is possible to have a partial thyroidectomy, in general doctors prefer to remove the complete thyroid in a total thyroidectomy, which will be more likely to prevent the cancer’s return at a later date. A partial thyroidectomy is likely to be carried out only for a papillary or follicular cancer that is less than a centimetre in width – and technically still in T1 stage. For those cancers that have been diagnosed as being medullary or anaplastic thyroid cancers (as well as larger papillary and follicular tumours) it is more common to remove the whole of the thyroid. Complete removal of the thyroid will result in the patient then needing to take thyroid hormone replacement tablets on a daily basis.
As with all surgical operations, there is an element of risk involved in thyroid surgery, albeit a very small one. Reactions to anaesthetics, infections or excess bleeding remain potentially harmful but, generally, thyroid surgery is considered a relatively safe and predictable procedure. The most likely possible complications specific to thyroid surgery are damage to the laryngeal nerve, which might result in hoarseness for a little while or, in extreme cases, permanent damage and hypothyroidism, which would then be medically treated.
